Medical Records Information: The vast
majority of patients Dr. Stewart saw in 2015 - 2016 were given a
CD with a PDF file on it that included all of the records we
have available. This included all of the office notes from
the Sammamish Diabetes and Lipid Clinic, lab results we had, and
reports from radiology and other consultants. Most
patients who saw Dr. Stewart between 2007 and 2014 have already
had their records forwarded to another physician, or have a copy
of records we provided when they left the practice, and should
check before requesting records.
For the most part, we don't have records between 1983 and 2007,
though they may be requested from Swedish Physicians Pine Lake,
who owns all of the old Pine Lake Family Medicine records.
Archived records more than 10 years old of inactive patients are
often destroyed, so we offer no guarantee that these records
will be available from Swedish.
Patients needing records from Sammamish Diabetes and Lipid
Clinic from 2007 through 2016 may send a signed request via
Snail Mail to the address above, or may fax the request to
(425)689-0073. Although we will respond to email requests,
we need to confirm identify before sending records. Record
extraction is time-consuming, and can only be done personally by
Dr. Stewart at home, so please allow adequate time for request
processing, which may be delayed due to travel.
Have a Pre-Visit
Interview A computerized pre-visit interview will make
your visit much more efficient, and will reduce the chance that
anything significant will be overlooked. This service is still being
offered through Instant Medical History as of
10/03/2018, and may be worth printing out and taking to
your new physician before a visit.
